Obtaining a Library Card | Where Can I Use My Card? | Lost Cards | Library cards are free to all Cook Memorial Public Library District residents aged 4 years or older. Cook Memorial Public Library District includes most of Libertyville and Vernon Hills, as well as parts of Green Oaks, Mettawa, Indian Creek, and Mundelein (map). Obtaining a Library Card for a Child, 17 and younger Children 4 to 13 years old must be accompanied by a parent or guardian when applying for a card. The parent or guardian must show photo identification and proof of residency in the Library District. Youths aged 14 to 17 must show proof of residency or be accompanied by a parent or guardian who can show proof of residency. Parents are responsible for materials checked out on their children's (aged 4 to 17) library card. Cook Memorial Public Library District adheres to the State of Illinois' Library Records Confidentiality Act. See our Patron Records policy for more information. Your card can be used at any Cook Memorial Public Library District library. Your card will also be honored at all North Suburban Library System libraries according to their circulation rules. Registration is required at each library. We suggest you contact the library before your visit to learn their registration requirements and lending policies. If you lose your library card, report it missing immediately or you will be held responsible for any materials checked out on your card. Call 847-362-2330 to report a missing card. Replacement cards are available. Replacement cards are available at a cost of $2.00. Visit the Circulation desk at any of our locations with identification to obtain a replacement card. Library cards with special privileges are available for homeschool families, teachers, businesses, and temporary residents.
