Getting a Library Card

CMPLDLibraryCardsmallIf you live in the Cook Memorial Public Library District, apply for a library card at any of our locations. Library cards are valid for three years. All residents age 4 and older are eligible.

Check which library district your residence is in. 

What do I need to do?

  • Bring photo identification and proof that you reside in our library district.
  • Complete and sign a library card application form (available at the Checkout Desks and Bookmobile).

What is an acceptable proof of residence?

Must include your current name and address. Examples include:

    • Lease
    • Mortgage statement
    • Utility bill
    • Official mail with a recent postmark
    • Preprinted checks
    • Insurance card

Library Cards for Children (17 and younger)

Children 4 to 13 years old must be accompanied by a parent or guardian when applying for a card. The parent or guardian must show photo identification and proof of residency in the library district.

Youths aged 14 to 17 must show proof of residency with photo ID or be accompanied by a parent or guardian who can show proof of residency.

Parents are responsible for materials checked out on their children's (aged 4 to 17) library card.


Use Your Card at Other Libraries


You may use your card at other Illinois libraries.

Check with the local library at your destination. You'll need to register your card at the library you are visiting.



Visitors- Registering a Card from Another Illinois Library

To register a card from another Illinois library, bring your valid home public library card and a current photo ID to any of our Checkout Desks.

Your library card will remain active in our system for one year based on the expiration date given by the home library.

Please note that some items and services are reserved for Cook Memorial Public Library cardholders only.


Renewing Your Card

Cook Memorial Public Library District card: Bring your library card and photo ID, with proof of in-district residence, to the Checkout Desk at one of our locations.

Reciprocal borrower (out-of-district) card: Update your account at your home library district, then bring your card and photo ID with current address to the Checkout Desk at one of our locations.


Lost Your Card?

LostCard75x75If your card is missing, notify the library right away since you're responsible for any activity on your account.

Call 847-362-0438 or visit the Checkout Desk.

Replace your card for $2.



Renew75x75Renewals may be done in person, online in your library account, or by calling 847-362-0438.

Please have your library card and the items handy when you call.



Placing Holds

If an item in our catalog is checked out or unavailable, place a hold to get in the queue and be notified when it is ready to be picked up.

Use the library’s online catalog to place materials on hold by clicking the Request It link in an item’s record. Enter your library card number, password, and choose where you would like to pick up the item.

If you would like to use Cook Park’s Drive-up Window, choose Drive-Up Window at Cook Park when placing your hold. Otherwise the hold will be placed on the shelves inside the library.


Email Notifications

Email NotificationReceive courtesy notifications by email when:

  • holds are available for pick-up
  • items are due soon
  • items are long overdue

To register or change your email address, fill out this form or speak with a staff member at the Checkout Desk.

Overdue notices are sent as a courtesy.

If you believe an error has occurred, please call our Checkout Desk at 847-362-0438 and a staff member will assist you.

  • Overdue fines exceeding $25 must be paid prior to checkout. Payment for fines is non-refundable.
  • Replacement of lost/damaged materials shall be at list price plus a non-refundable $5 processing fee.
  • Items are billed at 45 days overdue and must be paid or returned prior to checkout. Bill payments will be refunded, minus the processing fee, if the items are returned within 6 months.
  • Accounts may be referred to a collection agency at 60 days overdue, adding a $10 charge.
