Getting a Library Card
If you live in the Cook Memorial Public Library District, apply for a library card at the Aspen Drive or Cook Park Library Checkout Desks. Library cards are valid for three years. Check which library district your residence is in.
If you're in district,
- Bring photo identification and proof that you reside in our library district.
- Complete and sign a library card application form.
Your library card will also be honored at most other public libraries in Illinois. Please check with other libraries to learn their registration requirements and lending policies before your visit.
For Parents: Obtaining a Library Card for a Child (17 and younger)
Children 4 to 13 years old must be accompanied by a parent or guardian when applying for a card. The parent or guardian must show photo identification and proof of residency in the library district.
Youths aged 14 to 17 must show proof of residency with photo ID or be accompanied by a parent or guardian who can show proof of residency.
Parents are responsible for materials checked out on their children's (aged 4 to 17) library card.
Registering a Card from Another Illinois Library
We welcome visitors from outside our library district. Use many of our materials and services by registering your home public library card with us.
To register another library card, bring in your valid home public library card and a current photo ID to any of our Checkout Desks. Your library card will remain active in our system for one year based on the expiration date given by the home library.
Please note that some items and services are reserved for Cook Memorial Public Library cardholders only.
Renewing Your Card
Cook Memorial Public Library District card: Bring your library card and photo ID, with proof of in-district residence, to the Checkout Desk at one of our locations.
Reciprocal borrower (out-of-district) card: Update your account at your home library district, then bring your card and photo ID with current address to the Checkout Desk at one of our locations.

Acceptable Proof of Residence
Photo ID and one of the following, which must include your current name and address:
- Mortgage statement
- Utility bill
- Official mail with a recent postmark
- Preprinted checks
- Insurance card
Lost Your Card?
Treat it like a credit card: If you’ve lost your library card, report it missing immediately!
Otherwise, you could be held responsible for any materials checked out on your card.
Replacement cards are available. Bring identification to the checkout desk at either of our locations to obtain a new library card. Cost is $2.
Renewals may be done in person, online in your library account, or by calling 847-362-0438. Please have your library card and the items handy when you call.
Renewals on most items are available for two additional loan periods if no holds are pending. Hot Picks may not be renewed.
If an item in our catalog is checked out or unavailable, you can place a hold to get in the queue and be notified when it is ready to be picked up.
Use the library’s online catalog to place materials on hold by clicking the Request It link in an item’s record. You will be prompted to enter your library card number, password, and choose where you would like to pick up the item.
If you would like to use Cook Park’s Drive-up Window, choose “Drive-Up Window at Cook Park” when placing your hold. Otherwise the hold will be placed on the shelves inside the library.
Receive courtesy notifications by email when:
- holds are available for pick-up
- items are due soon
- items are long overdue
To register or change your email address, fill out this form or bring your library card to one of our locations and speak with a staff member at the Checkout Desk.
Overdue notices are sent as a courtesy. There are rare occasions when errors are made on patron accounts. If you believe an error has occurred, please call our Checkout Desk at 847-362-0438 and a staff member will assist you.

- Replacement of lost or damaged materials shall be at list price plus a processing fee of $5.
- Overdue charges must be paid prior to checkout if the total exceeds $25.
