All residents of Cook Memorial Library District aged 4 and older are eligible for a library card. Current proof of residency must be presented to obtain a library card. A parent or guardian’s signature is required on applications for children under 14. Library cards must be presented to check out or renew library materials.
Cards issued to residents of this library district may be used at most public libraries in Illinois. Additional ID may be required.
You may check out as many items as you are willing to be responsible for, subject to the following limitations:
- 2 Hot Pick DVDs
- 2 Video Games
- 10 DVDs (includes Hot Picks)
- 10 Magazines
- 5 MyMediaMall titles
- 40 Audio CDs
4 Week Loans:
- LINKin Materials
3 Week Loans:
- Series DVDs
- Nonfiction DVDs
1 Week Loans:
- All other DVDs
- Video Games

- Microfilm materials
Renewals on most items are available for two additional loan periods if no reserve is pending. Materials may also be renewed by phone or via the library’s website. If renewing by phone, the library card and barcode numbers of the item to be renewed must be provided. In person, a library card or valid identification must be presented. Hot Picks may not be renewed. LINKin items may be renewed once, if not on hold at the library of origin.
$.10 per day on overdue books and magazines – Maximum fine $5
$.10 per day on overdue CDs, CD-ROMs, and audiobooks – Maximum fine $5
$1 per day on all overdue DVDs and Video Games– Maximum fine $10
$1 per hour on overdue Overnight Reference Books – Maximum fine $5
Replacement of lost or damaged materials shall be at list price plus a processing fee of $5.
Overdue charges must be paid prior to checkout if the total exceeds $10.
Payment accepted for fines is nonrefundable.
1st overdue notices for DVDs and Video Games will be mailed or emailed when items are 3 days overdue. Notices for all other materials will be sent at 15 days overdue. 2nd overdue notices are mailed at 29 days overdue, and card privileges suspended at that time. Letters are sent at 45 days for items not returned. Accounts are referred to a collection agency when materials are 60 days overdue, and an additional $10 processing fee will be added to the account. Notices are not sent for outstanding fines.
All circulating items with the exception of Hot Picks may be placed on reserve, with a total limit of 50 reserves. No more than 10 items may be reserved through Interlibrary Loan Service (ILL) at any one time. Interlibrary Loan Services and LINKin requests are restricted to residents of the Cook Memorial Public Library District.
