Get a Library Card

If you live in the Cook Memorial Public Library District, apply for a library card at any of our locations.

  • All residents aged three and up are eligible.
  • Library cards are valid for three years.

Library Cards for Children

Ages 3-13

Children from age 3 to 13 must be accompanied by a parent or guardian when applying for a card. The parent or guardian must show photo identification and proof of residency in the Library District.

Ages 14-17

Youths aged 14 to 17 must show proof of residency with a photo ID or be accompanied by a parent or guardian who can show proof of residency.

Library Card Policy Update

Beginning June 1, 2026, District residents of all ages may apply for a library card — even the littlest library users. By removing the minimum age requirement, we’re expanding access and making it easier for families to connect with the Library from the very beginning.  

Many families already bring babies and toddlers to story times, check out board books, and read together at home. This change aligns our card policy with how families are already using the Library and encourages those early connections from day one. 

The Library is here to support individuals and families at every stage of life, but ultimately, decisions about a child’s card and use of materials remain at the discretion of a parent or caregiver. 

Do I need my own library card to get one for my child? 

No, you don’t need your own library card to sign your child up. Just keep in mind that parents or guardians are responsible for any items checked out on their child’s card until age 17. 

What do I need to get a card for myself or my child? 

When you come into the library, please bring your photo ID with official information that shows your current name and address. 

What if my ID does not have my current address? 

For address verification, please see the FAQ section below for a list of approved documents. 


Registering a Card from Another Illinois Library

If you have a library card from another Illinois library you can register it with us and check out our materials. To register, bring your valid home public library card and a current photo ID to any of our Check Out Desks.

Please note that the following Library offerings are reserved for Cook Memorial Public Library cardholders only:

  • Requesting items from other libraries via ILL/Linkin (exception for Vernon Area Public Library patrons with Vernon Hills addresses)
  • Home access to online databases
  • eLibrary
  • Notary service
  • Orion StarBlast telescope
  • Select Library events when noted

Library Card FAQ

Check the Illinois Public Library Service Area Populations map to verify which library district you reside in.

A photo ID with official information that shows current name and address. A driver’s license or state ID card with current address is sufficient.

For address verification, please provide one of the following documents in addition to your photo ID.

  • Utility bill
  • Lease
  • Mortgage statement
  • Pre-printed checks or bank statement
  • Voter registration card
  • Insurance card with address
  • Official mail with recent postmark

Visit the Check Out Desk at either library location or the Bookmobile to complete and sign a library card application form. You will be issued a library card. That’s it!

Cook Memorial Public Library District card:
Bring your library card and photo ID, with proof of in-district residence, to the Checkout Desk at one of our locations.

Reciprocal borrower (out-of-district) card:
Update your account at your home library district, then bring your card and photo ID with your current address to the Checkout Desk at one of our locations.

Replace card fee: $2
